Plasma bupivacaine concentrations during combined regional and general anaesthesia for resection and reconstruction of head and neck carcinomata.

چکیده
Venous plasma bupivacaine concentrations were measured in patients undergoing resection and reconstruction surgery for intra-oral malignancy. Vth cranial nerve and cervical plexus blocks were performed using 0.5% bupivacaine. Total dosage was higher than recommended by U.K. manufacturers. Mean peak venous plasma concentrations were higher than previously recorded. A significant different was found in the peak concentrations when adrenaline was included. The significance of these results in relation to systemic toxicity and recommended dosage is discussed.

Transoral Resection of Giant Parapharyngeal Space Tumors via a Combined Surgical Approach
Introduction: Parapharyngeal space (PPS) tumors account for 0.5% of the head and neck neoplasms. Based on the evidence, 80% of these tumors are of a benign nature. Surgical excision is the treatment of choice for this condition. The present study was conducted to propose transoral resection as an efficient way to excise the benign well-defined tumors of the PPS. Materials and Methods: Thi...

Background: Reconstruction by free tissue transfer and microvascular anastomosis can provide a reliable repair for tissue defects in head and neck surgeries. During this study, we evaluated the clinical characteristics and outcomes of reconstructive surgery by the use of free flaps for defects resulting from head and neck cancers. Methods: This quasi-experimental study included 29 patients ha...
متن کاملCase Report of an open Neck Procedure Complication Associated with Laryngeal Mask Airway Use
Introduction: The laryngeal mask airway (LMA) is a safe method to establish airway control during general anaesthesia. In otolaryngology surgery, the use of a LMA is well established in ear surgery; however, the use of a LMA during open neck procedures remains controversial. We report a case in which the pharyngeal distortion by the LMA cuff resulted in an iatrogenic complication. Case Report...
متن کاملPlasma concentrations of bupivacaine and two of its metabolites during continuous interscalene brachial plexus block.
An interscalene brachial plexus block was performed via a catheter with 20-28 ml of 0.75% bupivacaine plus adrenaline for surgery of the shoulder region in 12 patients. Constant infusion of 0.25% bupivacaine 0.25 mg kg-1 h-1 was continued for 24 h. During surgery light general anaesthesia, without analgesics, was maintained. Plasma concentrations of total and unbound (free fraction) bupivacaine...
متن کاملCombined Epidural-General Anesthesia (CEGA) In Patients Undergoing Pancreatic Surgery: Comparison Between Bupivacaine 0.125% And 0.25%
Background: Major abdominal surgery results in homodynamic instability mainly due to endogenous prostacyclin release, bleeding, major intercompartemental fluid shift and the hormonal surgical response. This study compared the effects of low thoracic epidural anesthesia with 0.125% and 0.25% bupivacaine on haemodynamic variables, sevoflurane requirements, and stress hormone responses during panc...
